A loved one returning home from the hospital, respite center, emergency department, or skilled nursing facility is a joyous occasion. However, it can also be fraught with worry and uncertainty, especially if the person is still recovering from illness, injury, or surgery, or is diagnosed with chronic conditions. Post discharge, the patient is suddenly without ongoing medical supervision and may need assistance with the Activities of Daily Living, such as bathing, grooming, medication management, meal preparation, and more.

Senior Living Advisors

2nd Family’s Senior Care Advisors will help you identify if home care is best for the recovering patient. They’ll answer all of your questions, tell you about our comprehensive transition care program, and connect you with your 2nd Family transitional health care team.

RN Care Coordinator

Your RN Care Coordinator will oversee all aspects of your loved one’s care, including managing your transition home, discharge planning, care plan development, and caregiver supervision. The Coordinator will also liaise with your primary medical team to keep them apprised of your loved one’s ongoing well-being.

Trained and Certified Caregivers

2nd Family caregivers are professionally certified, trained, and qualified to provide safe quality patient care. Each caregiver holds a valid CNA or GNA certificate*, is thoroughly screened with background checks, and is bonded and insured through our company. Our goal is to connect you with safe, quality care practices that focus on your loved one’s specific needs.

What’s included in the transition care program?

Not all adults need to transition to a nursing home or long-term care facility. We believe that home is the best place to be for the patient, surrounded by familiar comforts and decades of happy memories. With our FamilyFirst Transition Program, your loved one will enjoy a wonderful patient-centered care experience for as long as it’s needed.

Comprehensive health care plan

2nd Family caregivers work as an extension of your loved one’s primary healthcare team. We communicate, collaborate, and coordinate with doctors, therapists, social workers, and patient families to create and maintain a complete healthcare plan. We’ll keep everyone updated on recovery progress and if any part of the plan needs updating.

Personal Care

Our caregivers will assist with personal care tasks such as bathing, grooming, dressing, and toileting so the person feels fresh and comfortable. We perform these duties with care and patience while preserving your loved one’s dignity at all times.

Light housekeeping

We take on light housekeeping duties by assisting with laundry, dishes, floor cleaning, and more. Our goal is to help maintain a safe, clean environment while your loved one focuses on recovery.

Meal preparation 

2nd Family caregivers help your older relative access proper nutrition through meal planning, grocery shopping, and meal preparation services. We cater meal plans to special dietary needs that suit every nutritional, allergic, or religious requirement.

Transportation and accompaniment

We’ll help your loved one get around with our transportation services, whether it’s to a medical appointment, shopping, or social event. Our caregivers will get that special person to each destination and home again safe and sound.

Medication reminders

Our team understands the importance of taking the proper dosage at the right time for your loved one’s recovery and long-term well-being. We’ll help manage the person’s medication to ensure all prescription and non-prescription medications are taken correctly.

Alzheimer’s disease and dementia care

A diagnosis of Alzheimer’s disease or dementia adds an extra layer of care challenges for older people. 2nd Family caregivers have gone through extensive, specialized training to support older adults with progressive cognitive conditions and help families cope with the effects of the disease.
