Trained and Certified Caregivers
2nd Family caregivers are professionally certified, trained, and qualified to provide safe quality patient care. Each caregiver holds a valid CNA or GNA certificate, is thoroughly screened with background checks, and is bonded and insured through our company.

2nd Family caregivers work as an extension of your loved one’s primary healthcare team. We coordinate with doctors, therapists, social workers, and families to maintain a complete healthcare plan and keep everyone updated on progress.
Personal Care
Our caregivers assist with personal care tasks such as bathing, grooming, dressing, and toileting so they feel fresh and comfortable while preserving a sense of dignity at all times.
Light housekeeping
We assist with laundry, dishes, floor cleaning, and more to help maintain a safe, clean environment while your loved one focuses on recovery.
Meal preparation
2nd Family caregivers support proper nutrition through meal planning, grocery shopping, and meal preparation services, tailoring meals to any dietary needs, including allergies and religious preferences.
Transportation and accompaniment
We provide safe transportation services for medical appointments, errands, and social outings, ensuring your loved one gets to and from each destination comfortably.
Medication reminders
We ensure your loved one takes the right medications at the right time by managing all prescription and non-prescription doses accurately.

Who pays for transitional care management?
Transitional care services are typically paid for through private pay, long-term care insurance, or veterans’ benefits such as Aid and Attendance. While Medicare may cover certain post-discharge medical follow-ups, it does not usually cover non-medical in-home caregiving services.
2nd Family’s care advisors will help you understand your options and explore available benefits, insurance coverage, or alternative funding sources to make care more affordable.
What documentation is required for transitional care management?
To create a safe and effective transitional care plan, families are usually asked to provide hospital or facility discharge paperwork, a current medication list, physician or specialist contact information, and any recommended follow-up care instructions. If available, therapy schedules, dietary guidelines, or mobility restrictions are also helpful.
