At 2nd Family, our approach to senior care means our services are designed to make the care experience easier and more enjoyable for our clients and their families. We manage the entire home care process for you right from the beginning. This way, you can worry less about providing care and spend more time creating memories with your elderly loved one.

Step One RN Care Coordinators

The structure of our approach to senior is unique. Our RN Care Coordinators are case management experts who help oversee your family’s home care experience. The RN Care Coordinator will be your single point of contact for all questions or concerns. This eliminates your need to juggle multiple connections.

Your RN Care Coordinator will be a Registered Nurse or a certified geriatric care professional. This care professional will help develop a customized care plan for your loved one based on the level of care required. We can also include primary health providers and coordinate with the caregivers to ensure they completely understand your needs.

The coordinator will be in constant contact with the caregivers and personally visit your home every three months to ensure that our care program is still meeting your needs as long as your loved one lives at home. She can tweak the schedule, activities, or services to ensure you get the most out of your care experience.

The RN Care Coordinator will provide valuable feedback to our care schedulers, who use our accurate caregiver matching system to find the right group of caregivers for your loved one.

Having a caregiver that complements your loved one’s personal attributes is also critical to having a positive care experience. For example, your family member might like someone chatty and outgoing. In contrast, others might prefer a caregiver who’s quiet and reserved.

We use a unique, detailed process that gathers information about your loved one for accurate matching with the right caregivers. This approach significantly increases the chance of a fulfilling, enjoyable care experience for everyone involved.

Step FourOngoing Caregiver Training

At the heart of a positive care experience is the strong, trusting bond between our caregiver and your loved one. Our rigorous onboarding process helps ensure our clients receive care exceeding your expectations.

Every 2nd family caregiver must pass a criminal background check, have at least three related job references, a valid driver’s license, and CPR certification.

All 2nd Family caregivers have successfully completed specialized training in:

CPR (Cardiopulmonary Resuscitation)

Certified Nursing Assistant certification*

2nd Family care policies, procedures, and practices

Many of our caregivers are offered ongoing learning opportunities. This additional training allows our caregivers to stay up to date on industry-leading resources for Alzheimer’s disease and dementia, diabetes, Parkinson’s disease, and more.

We require 2nd Family caregivers to be among the best on a professional level. However, we also focus on retaining people who bring happiness and sunny dispositions to their work every day. We want your loved one to look forward to our visits and enjoy every moment of our home care services.

Family Portal
Transparency is at the heart of our approach to senior care. Our family portal offers a secure, easy-to-use resource for families to stay updated on their loved one’s care plan with just a few keystrokes. This technology provides complete transparency, maximum accountability, and open communication with your RN Care Coordinator through the online message board.

Register multiple family users for each client account, so everyone can have their own individual login information. Create as many accounts as you like so the whole family can be involved in the care plan and work together to create an optimal care experience for everyone.

Care plan information
Access all care plan details and paperwork signed during the initial consultation in one place whenever you want to review information or ask questions.

Caregiver scheduling
Quickly view your loved one’s upcoming caregiver schedule along with a picture of the caregiver for any listed shift, eliminating any unexpected surprises.

Past shift data
Quickly review important details about a past shift, including when the caregiver arrived, what tasks were performed, and when the caregiver left, giving you a complete view of how the care plan is being delivered.

Caregiver Portal
Our caregivers also use innovative technology to improve the customer experience through our caregiver portal, where they can access the latest healthcare resources and be instantly updated about any changes to your loved one’s care plan.

Caregivers are also required to use their GPS Mobile Timecard to log completed caregiving tasks, such as assistance provided with personal care and the Activities of Daily Living (ADLs). Your loved one’s file will continually be updated so all caregivers, medical professionals, and family members are always kept in the loop.
